When it's time for your next appointment, here are some helpful tips to keep in mind:

Please be sure to have the following items with you and arrive 15 minutes before your scheduled appointment time. 

  • Medical and vision insurance cards
  • List of any medications you may be taking
  • Driver’s License/Photo ID
  • Form of payment

There are two types of insurance that will pay for your eye care services and optical products.

  1. Vision: Only covers routine vision wellness exams and materials (glasses and/or contact lenses.) Vision plans do not cover medical eye care (the diagnosis, management, or treatment of eye health problems.)                                                                                Please note: your insurance may not cover the contact lens evaluation portion of your exam, and/or contact lens fitting, in which it is your responsibility.                                                    
  2. Medical: Must be used for medical eye care. Any medical diagnoses found, your exam will be sent to your medical insurance.                                                                                          Please note: Highmark Freedom Blue and Medicare do not cover a refraction. That amount will be your responsibility.  (The refraction is a vision test which determines the need for an eyeglass prescription).

If you have both types of insurance, it may be necessary for us to bill some services to medical and some services to vision.

If some fees are not paid by your insurance, such as deductible, co-pays/co-insurance, or non-covered services, that amount then becomes your responsibility.
